What the assessment looks at
How your condition affects everyday life
Brittany’s assessment focuses on how a mental health condition affects your daily functioning, so VAC has a clear, professional picture of your situation alongside the rest of your application.
Home & daily routines
Sleep, self-care, household tasks and managing day-to-day responsibilities.
Work & concentration
Focus, memory, energy and your ability to keep up with your responsabilities.
Relationships
Connection with partners, children, family and friends.
Community & activities
Getting out, driving, social settings and the activities that matter to you.
Clear expectations
What this assessment does, and doesn’t do
We want you to know exactly what to expect before you book.
The assessment does
- Document your mental health condition and its impact on your daily functioning
- Complete the VAC medical questionnaire VAC has asked for
- Give VAC clear, professional information to consider with your application or reassessment
The assessment doesn’t
- Guarantee a VAC decision. VAC decides all entitlement and benefit amounts
- Replace ongoing therapy or treatment, which MindSpa can also help with
- Provide emergency or crisis care (see crisis supports below)

Cost & billing
Clear billing. Your benefit stays yours.
We keep billing simple and transparent, so you can focus on telling your story.
Eligibility and coverage are determined by VAC. If you’re unsure whether your assessment is covered, call us before booking and we’ll help you check.
Billed directly to VAC
When VAC has requested a medical questionnaire, eligible and authorized services are billed directly to VAC or Medavie Blue Cross.
No cost to you for VAC-requested assessments

Questions
Frequently asked questions
Still unsure? Call us at 613.592.1001 and we’ll walk you through it.
Do I need to contact VAC before booking?
Yes. VAC asks Veterans to request the medical questionnaire first, and providers should complete it only once VAC has sent instructions. This also ensures the assessment can be billed directly to VAC.
Can a Nurse Practitioner complete the mental health questionnaire?
Yes. VAC lists Nurse Practitioners among the health professionals who can complete its Psychiatric/Psychological Condition(s) medical questionnaire.
Will this assessment guarantee my benefit?
No. The assessment documents your condition and its impact on daily functioning. VAC reviews your full application and makes all decisions about entitlement and benefit amounts.
Can this help with a reassessment?
Yes. If your condition has changed, you can ask VAC for a reassessment, and Brittany can complete the assessment VAC requests.
What should I bring?
Your VAC medical questionnaire, VAC’s signed consent form allowing VAC to collect your information, and any relevant medical records you’d like to share.
Is MindSpa part of Veterans Affairs Canada?
No. MindSpa is an independent mental health centre and isn’t affiliated with VAC. We provide the assessment; VAC makes the decision.
